How to become a customer:
There are two ways to set up an account with us, and both are very simple.
1) Net-30 (Invoice)
Fill out our one-page terms application. We can usually get new facilities set up and product shipped the same day we receive it.
Email the completed form to info@urgentsurgerysolutions.com (or your representative), or fax it to us.
If your location needs terms other than our default Net-30, we can usually accommodate.
2) Credit Card
Processing fee: 1.5% for Visa/Mastercard, 3.5% for American Express. Complete this one-page form, or call your rep if you prefer to provide the card by phone (480-478-6450 → Sales). We only charge the card after product ships and email the receipt to you.
If you later switch to Net-30 terms, email accounting@urgentsurgerysolutions.com or tell your representative.
*Note: If you’re part of a health system/corporation, or have payment history with related locations, you may be able to bypass the Net-30 paperwork—especially if we already bill your corporate accounting.
For any location we ship to, we require a copy of one license (facility, pharmacy, MD, or DEA).
